Case Management

Community Case Management Services at Group Main Stream (GMS) are designed to meet the needs of consumers through an individualized approach. GMS serves individuals with Intellectual Disabilities and Autism Spectrum Disorders who live on their own, with family, in a staffed setting or Shared Living Option (SLO.) Our Case Management Department is committed to supporting each person served by assisting in the development or maintenance of a comprehensive spectrum of services. Individuals are provided with options and encouraged to make independent choices which best meet their needs and desires. All services are designed to encourage the highest level of home and community independence.

GMS is one of the few agencies in the State of Maine providing Case Management services by individuals fluent in ASL. As an agency GMS is committed to providing the highest quality services for people who are Deaf, Hard of Hearing  or experience communication barriers. Everyone receiving case management services will have a Person Centered Plan (PCP) either written or reviewed by our highly trained team of Case Managers. A PCP describes how services and resources will be combined to support goals and needs. All services are designed to encourage the highest level of home and community independence.

In addition, GMS offers representative payee services and general financial management assistance.

Community Case Management Services are provided from our Westbrook office in the following counties:

  • Cumberland

  • Southern Kennebec

  • Sagadahoc

  • Knox

  • Lincoln

  • Southern Oxford
